TUTORING WAIVER FORM
CHESAPEAKE, VA
Thank you for choosing to spend time at Our Sensory Farm, an agritourism site in Chesapeake Virginia. Before your visit, there are a few things you should know. Please read the following information carefully, then complete the form and sign below acknowledging your understanding and willingness to comply.
What to Know: In Virginia, agritourism is defined as any activity carried out on a farm or ranch that allows members of the general public, for recreational, entertainment, or educational purposes, to view or enjoy rural activities, including farming, wineries, ranching, historical, cultural, harvest-your-own activities, or natural activities and attractions. For most people, agricultural tourism refers to a visit to a working farm or any agricultural, horticultural, or agribusiness operation in order to enjoy, be educated, or become actively involved in the activities of the farm or operation – getting a true farm experience. Since Our Sensory Farm meets this definition, under Virginia law, there is no liability for an injury to or death of a participant in an agritourism activity conducted at this agritourism location if such injury or death results from the inherent risks of the agritourism activity. Inherent risks of agritourism activities include, among others, risks of injury inherent to land, equipment, and animals, as well as the potential for you to act in a negligent manner that may contribute to your injury or death. You are assuming the risk of participating in this agritourism activity. During your visit, please remain within the designated public spaces (your farm guide will share specifics with you upon arrival).
What to Wear/Bring: Remember to check the weather prior to your visit and dress appropriately! Closed toed shoes are mandatory! We recommend dressing in layers and wearing a hat. You may also choose to bring bug spray, sunscreen and a refillable water bottle.
What to Expect: Upon arriving at Our Sensory Farm, please park in the area designated by the cones to the right of the driveway. Follow the signs along the left of the proerty to the farm. We offer an outdoor ADA accessible public restroom as well as a handwashing station. There is a shade structure and picnic table seating available. Drinking water will be provided to refill your water bottles.
Media Release: Photos and videos may be taken by guides, photographers and other guests during your time at Our Sensory Farm. By signing this form, you are granting permission for your likeness to be used in a photograph, video, or other digital media.
Rescheduling and Cancellations: If you need to reschedule or cancel a tutoring session, please do so no less than 24 hours prior to the scheduled date/time using our online We Play Tutoring Portal. If you need to reschedule or cancel a social club meetup or field trip reservation, please do so no less than 48 hours prior to the scheduled date/time using our online We Play Tutoring Portal. If you need to cancel within 24 hours of a scheduled event, please email Shannon Wells at info@oursensoryfarm.com.
Unused Tutoring Credits: We Play Tutoring Credits do not expire and can be used during our fall, winter and spring sessions. Tutoring credits are nonrefundable, but can be transferred to other school-age learners within the household.
Nursing Services: Nursing services will not be available during tutoring sessions, social club meetups, or field trips, as We Play does not have a nurse on staff. However, we are here to help in any way we can and encourage families to discuss medical/health concerns with us prior to registering for events.
Attendance: We ask that learners attend a minimum of one tutoring session per month to help maintain connections and make progress toward our goals. Learners who are enrolled in We Play and attend at least one tutoring session per month are welcome to register for our Social Club Meetups and Field Trips.
Alternate Indoor Locations: With the safety and well-being of our learners and their families in mind, alternate indoor locations have been assigned to each tutoring session. These safe spaces within our community can be utilized during times of inclement weather, extreme seasonal temperatures, or reduced daylight. You will receive a notification by 8 am on the day of your scheduled tutoring session when a location change is necessary. If you ever have questions about the location of a We Play event, please email Shannon at info@oursensoryfarm.com.
Singular Tutoring: My goal is to have multiple learners attend every tutoring session. With best practice in mind, if your learner is the only individual registered for a class, I will contact you at least 24 hours prior to the scheduled date with the option of hosting a chaperoned tutoring session (caregiver stays) or rescheduling your learner.
